Comprehensive Health Network (CHN) Chronic Disease Management

More than 40% of the U.S. population has one or more chronic disease. We are seeing a significant increase in people with multiple chronic diseases. Although chronic diseases cannot typically be cured, we believe with the right medical care and patient commitment these diseases can often be managed.

Chronic Disease Management includes many different components. At CHN we work with you and your family in:

CHN is proud to be able to say that our nurses have received advanced training and are certified in Chronic Disease Management. This training makes us a perfect partner on “Your Health Care Team”. We work with you and your physician – creating a plan of care that is specifically designed to help you manage your illness.

Educating our patients and their families is a major part of what we do. Our goal is to empower you with the knowledge to improve/maintain your quality of life while avoiding unnecessary E.R. visits and re-hospitalizations.

Home Care After Surgery

When you are discharged from the hospital or rehab facility after surgery, you may need home care.

Patients and their families are finding that they are going home with wounds that need dressing changes, medications that need to be given by IV or injection, exercises that need to be done, etc. This can be overwhelming – particularly at first.

When you are admitted to the hospital, simply tell the nurse or discharge planner that you want CHN for your home care. WE WILL TAKE IT FROM THERE!

You may also contact us prior to your surgery to arrange for care. WE WILL TAKE IT FROM THERE!

Wound Care

CHN nurses have extensive training in managing wounds – surgical wounds, diabetic ulcers, pressure ulcers, and stasis ulcers. We use the latest, most advanced wound care products to promote rapid healing and pain management.

All nurses are certified to use wound Vacuum Assisted Closure therapy to promote increased healing.

Our physical therapists provide pulse lavage therapy to promote wound debridement.

Our Certified Wound Care Specialist Nurse reviews all wound care treatments and makes recommendations.

Many times we are able to teach you and your family to assist in your wound care.

Infusion Therapy

Our Registered Nurses are able to administer intravenous fluids and medications in your home. We utilize the latest in IV equipment and are often able to teach you or your family to administer your own treatment.

We will arrange for all equipment and supplies to be delivered to your home.

Telehealth

CHN offers a state-of-the-art telehealth program at no cost to our patients. Monitoring units are placed in your home. Your nurse will teach you how to check your blood pressure, pulse, oxygen level, weight, and blood sugar if you are a diabetic. OUR GOAL IS TO IDENTIFY SYMPTOMS BEFORE THEY BECOME A PROBLEM.

Your results are transmitted to our office via the phone line. Trained nurses review your readings, keep your physician informed, and contact you with treatment changes.

You, your physician and your family (with your permission) are able to view your readings on-line.

We have seen that patients with chronic diseases such as high blood pressure, congestive heart failure, diabetes, and COPD are better able to manage their disease while using our telehealth program. Telehealth has been proven to decrease unnecessary E.R. visits and re-hospitalizations while providing a better quality to life for the patient.

MRDD Services

CHN has nurses trained to specialize in caring for the developmentally disabled individuals in our community. We provide services at the local workshops, preschool, and in your home.

Bringing Additional Services To You

CHN can facilitate bringing services to you when you are unable to leave your home:

• In-Home Podiatry
• In-Home X-Ray
• In-Home Physician Visits
• In-Home Therapeutic Medical Massage
