Each of these companies may have numerous insurance plans or products, which Family Health may or may not be contracted with. The best way to know what is covered by your plan is to call your insurance provider. The number is usually found on the back of your insurance card. You may use Family Health’s Tax ID 34-1119524 when calling to verify coverage.
Family Health continues to offer different payment options to help those in need. To help provide the most affordable access and care possible, we offer a 20%-30% prompt pay discount on the date of service, as well as a Sliding Fee Discount Program for patients meeting certain income criteria and family size guidelines. If you have questions about either one of these options, please call your provider’s front office.
