Qualifications:
- Doctor of Pharmacy (PharmD) or equivalent degree from an accredited school of pharmacy.
- Current and valid pharmacist license in the state of Ohio.
- Prior experience in a 340B pharmacy setting or FQHC preferred, not required.
- Strong understanding of 340B program compliance and operations, preferred not required.
- Excellent communication, organizational, and patient counseling skills.

Position Summary:
Family Health is seeking a dedicated and detail-oriented Quality Assurance Nurse (RN) to support our commitment to clinical quality, patient safety, regulatory compliance, performance improvement, and population health.
The Quality Assurance Nurse applies professional nursing knowledge, quality improvement principles, evidence-based practices, and data-driven methodologies to monitor, evaluate, and improve the quality, safety, effectiveness, and efficiency of patient care
This position works collaboratively with the Director of Quality and Risk, medical providers, nursing leadership, clinical staff, operations, information technology, health information management, and other members of the health care team. The Quality Assurance Nurse helps identify gaps in care, monitor quality performance, improve clinical workflows, promote evidence-based practices, and support compliance with applicable federal, state, payer, accreditation, and organizational requirements.
Key Responsibilities:
The Quality Assurance Nurse provides clinical and analytical support for quality measures and reporting requirements, including applicable:
- HRSA/UDS programs
- CMS quality programs
- Patient-Centered Medical Home (PCMH) initiatives
- Medicaid and Medicare programs
- Managed care and payer quality programs
- Regulatory, accreditation, and contractual requirements

Position Overview
Family Health is seeking a compassionate and skilled School-Based Therapist/Counselor to provide mental health services to students in a school setting. This full-time, school-year position (August through May) plays a vital role in supporting students’ emotional well-being, behavioral health, and academic success through counseling, crisis intervention, and collaboration with school staff and families.
Key Responsibilities
- Provide individual and group counseling to students addressing emotional, behavioral, and social concerns
- Conduct assessments, establish diagnoses, and develop individualized treatment plans
- Deliver crisis intervention and risk assessments as needed
- Collaborate with teachers, administrators, and school personnel to support student success
- Participate in IEP and Student Support Team meetings when appropriate
- Consult with parents/guardians regarding student progress and needs
- Provide psychoeducation and prevention programming (coping skills, emotional regulation, etc.)
- Maintain accurate, timely clinical documentation in the electronic medical record
- Coordinate referrals to community resources and ensure continuity of care
- Work as part of a multidisciplinary behavioral health team
Qualifications
- Master’s degree in Counseling, Social Work, Psychology, Marriage & Family Therapy, or related field
- Current Ohio licensure or license-eligible (LPC, LPCC, LSW, LISW, LMFT, or equivalent)
- Ability to diagnose and treat mental health conditions using DSM-5 criteria
- Strong clinical, communication, and organizational skills
- Experience working with children/adolescents preferred
