Comprehensive Health Network (CHN) recognizes this and respects your right to make your own decisions regarding your care.
You may need assistance as a result of an accident, illness, or surgery, or to fulfill your wish to remain in your home through the aging process. For whatever the reason you are looking for assistance, CHN hopes to be your agency of choice.

- Choosing CHN gives you the best of both worlds in home care. We provide both skilled and non-skilled care. This gives you the benefit of transitioning between skilled services and non-skilled services and back without ever leaving our agency.
- As a Medicare certified home care agency, we are held to the highest of state and federal standards of patient care.
- Our nurses and therapists provide quality skilled services and under their guidance our certified nurse aides are available to provide non-skilled personal care and homemaking services.

You are welcome to stop in our office anytime from 8:00 am – 4:30 pm, Monday through Friday, and meet with one of our staff to discuss services. We are also available to come to your hospital room, nursing home, or your home. CHN will provide patient-centered, quality care in your home, apartment or even if you live in an assisted living facility. If you are being discharged from a hospital, the discharge planner will work with you to arrange for home care. The hospital is required to give you a list of all home care agencies available in your area. You have the right to choose your home care provider.
CHN has been providing home care to the residents of Darke County and the surrounding communities since 1988. CHN is a nonprofit organization. Our office is located in Greenville. We live and work in the community. We are neighbors taking care of neighbors.
We are not only committed to meeting the home care needs of our community, we are also committed to keeping jobs in our community. We only employ experienced nurses, therapists and aides. All employees complete an extensive orientation before ever working alone. All employees are bonded, insured and covered under WC insurance. CHN completes reference checks for all employees; requires a health check clearance; and state/federal criminal background checks.
