Our nurses provide a multitude of direct care services, but we cannot emphasize enough the importance we place on teaching. We teach with every visit we make.
We see ourselves as your advocate. We will help you to understand your disease process, the importance of complying with your treatment regimen, to identify signs and symptoms of complications before they become a problem, to understand and administer your medications properly, and to maintain an appropriate diet and exercise program.
As your advocate, we will help you manage your medical needs. This is particularly important when you have multiple physicians.
We also give you guidance to help you understand when to call your CHN nurse versus your physician or 911.
Our ultimate goal is to assist you in regaining/maintaining the highest quality of life possible for you while avoiding unnecessary E.R. visits and re-hospitalizations.
