Our relationship with our parent company affords us the opportunity to access all of their services to help our patients remain safely in their home – including home visits by a physician or nurse practitioner for those patients that have difficulty getting to their physician’s office.

A registered nurse is available to come to your home within 24-hours of your referral to our agency.

There are a variety of reasons but are not limited to the following.

  • Have you been falling or losing your balance lately?
  • Is there confusion as to what medications you are supposed to be taking or when?
  • Has there been a recent hospitalization or emergency room visit the last few months?
  • Do you have an open sore that will not heal?
  • Do you need assistance with person care or light housekeeping?
  • Are you having surgery and want to recover in your home afterwards?
  • A new diagnosis or illness that you need assistance in learning to manage.

Yes, if you would prefer CHN services, just call our office and we will assist you in making the transfer to our agency.

CHN accepts tax-deductible donations as an investment in the health and safety of those living in our community. As a non-profit organization your gift is tax deductible and allows us to expand critical services, offer new programs, and support the needs of patients and families aging or recovering at home.
