Mission

Our mission is to build healthy lives together by cultivating practitioners who use principles of pharmaceutical care to develop, implement, and deliver comprehensive patient care services. We aim to optimize patient outcomes, place patients at the center of their healthcare, and educate both patients and other healthcare providers.

Purpose Statement

The Family Health Pharmacy Residency Program builds upon the Doctor of Pharmacy (PharmD) education to develop community-based pharmacist practitioners with diverse patient care, leadership, and education skills. Graduates are well-prepared to pursue advanced training opportunities, including postgraduate year two (PGY2) residencies and professional certifications.

Program Description

Residents gain experience providing pharmaceutical ambulatory care services to underserved populations in a community family practice setting. Areas of focus include diabetes, hypertension, asthma, COPD, harm reduction, and other chronic disease state management.

Our program allows residents to accelerate their clinical skills beyond entry-level professional competencies in patient-centered care, dispensing, and disease state management. Upon completion, residents will be able to:

  • Provide Medication Therapy Management (MTM) services.
  • Deliver community wellness and disease prevention programs.
  • Perform disease state management through collaborative practice agreements.
  • Assist and lead clinical program development.
  • Earn a teaching certificate (optional through Cedarville University).
  • Serve in a dispensing role.

Family Health Services offers a unique environment as a Federally Qualified Health Center (FQHC) with an in-house 340B pharmacy. Residents experience integrated, team-based care, project and people management, and a diverse patient population.

Application Requirements

  • Applicants must meet the following criteria:
  • United States Citizen
  • Licensed Pharmacist (or in process of licensure)
  • Curriculum Vitae (CV)
  • Letter of Intent
  • College Transcripts
  • Three Letters of Recommendation (using the ASHP standardized PhORCAS form)
  • On-site or Virtual Interview
