Payment & Insurance Options

The following list provides general information on our accepted insurance plans for Medical, Dental, Optometry, and Behavioral Health services and is subject to change at any time. Each of these companies may have numerous insurance plans or products, which Family Health may or may not be contracted with. The best way to know what is covered by your plan is to call your insurance provider. The number is usually found on the back of your insurance card. You may use Family Health’s Tax ID 34-1119524 when calling to verify coverage.

Family Health continues to offer different payment options to help those in need. To help provide the most affordable access and care possible, we offer a 20%-30% prompt pay discount on the date of service, as well as a Sliding Fee Discount Program for patients meeting certain income criteria and family size guidelines. If you have questions about either one of these options, please call your provider’s front office.

Health Insurance Marketplace

In 2010, the Patient Protection and Affordable Care Act (ACA), commonly known as ObamaCare, was signed into law. This law makes it possible for more families to have access to preventive, affordable health care.

Good Faith Estimate

A Good Faith Estimate is a projection of the typical charges for services provided at Family Health Services for those that are uninsured or underinsured and don’t qualify for our Sliding Fee Scale Discounts. The listed amounts are only an estimate; they are not an offer or contract for services. Final costs may vary from the estimates provided.

Sliding Discount Program

Family Health Services of Darke County, Inc. offers a program that reduces the cost of care for uninsured patients or can be applied to insurance deductibles for patients with insurance.

It is our mission to provide essential services regardless of ability to pay.

Discounts are offered depending upon household income and family size. A “Family” is one or more persons living in one dwelling place. This could include yourself, spouse or significant other, children under 18, parents, grandparents, adult children, etc. Ask any front desk receptionist for more clarification.
