Dietician

Good nutrition is a key factor in both preventing and improving conditions such as diabetes, heart disease, kidney disease, obesity, and high blood pressure. Our registered dietitian provides dietary and nutrition support to assist patients in achieving their optimal health goals.

Once the Dietitian receives a referral from the patient’s Primary Care Provider, the patient will be scheduled for a 60-minute initial consultation where the dietician will spend time assessing patient’s medical history, nutrition needs, and working with the patient to develop an individualized plan to help meet patient’s overall healthcare goals. 15-minute follow-up visits will be scheduled on a regular basis to monitor progress, break down barriers, and make changes to individualized plan as needed.
